Limited access to essential medicines (EMs) for cardiovascular disease (CVD) and diabetes is a major concern in low- and middle-income countries. link3 We aimed to generate data on availability, price and affordability of EMs for CVD and diabetes in India.
Using WHO/HAI survey methodology, we evaluated availability and prices of 23 EMs in 30 public sector facilities (government hospitals and semi-public/government-subsidised-discount-pharmacies (GSDPs)) and 60 private retail pharmacies across six districts in Kerala state, India (November 2018 - May 2019). Median Price Ratios (MPRs) were calculated by comparing consumer prices with international reference prices. We also analysed data (collected in July 2020) on six anti-hypertensive fixed-dose-combinations (FDCs) that were designated as 'essential' by the WHO in 2019.
Mean availability of surveyed generic EMs was 45.7% in government hospitals, 64.7% in GSDPs and 72.0% in private retail pharmacies. On average, the most-sold and highest-priced generics, respecget in both sectors. Lacaune ewes had their cervix evaluated by ultrasonography 12 hr after induced oestrus onset (Trial 1, n = 24) or 30 min before NSER (Trial 2, n = 17). Cervical rings were longitudinally evaluated and classified by their degree of misalignment on ultrasonography (DMUS) into DMUS-1-cervix rectilinear, DMUS-2-intermediate and DMUS-3-highly asymmetrical. For predicting cervical transposing, only DMUS-1 and DMUS-2 were considered suitable. Similar ranking was attributed to degree of misalignment on the cervical map (DMCM 1-3), established immediately before NSER, which was performed at days 6 to 7 after oestrus. In Trial 1, cervical retraction for NSER was not possible only in three ewes classified as DMUS-3 (3/14, 21.4%). No difference (p > .05) was observed in the cervical transposing rates between ewes with different DMUS (ranged from 80% to 100%). In Trial 2, DMUS-1 and DMUS-2 reached 100% of transposing, and the only DMUS-3 ewe has not been transposed. In Trial 1, the prediction performance for successful cervical transposing showed low sensitivity (45%) and no specificity due to a high incidence of false negatives (52%). However, in Trial 2, sensitivity and specificity were both 100%. The DMCM and DMUS were uncorrelated, probably due to cervical stretching required to perform NSER. In conclusion, cervical ultrasound assessment immediately before NSER was more efficient to predict the cervical transposing than at induced oestrus, allowing the classification and selection of ewes eligible for NSER.Streptococcus agalactiae is a Gram-positive facultative intracellular bacterium that leads to severe economic loss of tilapia worldwide. Previous studies demonstrated that CD40 contributes to host protection against intracellular injection. In this study, CD40 was characterized from Nile tilapia (Oreochromis niloticus), named OnCD40. Sequence analysis showed that open reading frame of OnCD40 was 933 bp, containing a single peptide, a transmembrane domain and four cysteine-rich domains. The qRT-PCR revealed that OnCD40 was expressed in all examined tissues with the most abundant ones in spleen and thymus. After S. agalactiae stimulation, the expression of OnCD40 was significantly induced in most of the detected organs. Moreover, OnCD40-overexpressing fish elicited significant protection against subsequent S. agalactiae challenge; approximately 10000-fold fewer bacteria were detected in spleen of OnCD40-overexpressing fish in comparison with control fish. Thus, CD40 had protecting function in Nile tilapia against intracellular pathogens.Responding to the commentaries on a recent paper on the elusiveness of representativeness in general population alcohol surveys, we can summarise that there is agreement that the status quo of current alcohol surveys is scientifically no longer defensible. Current surveys cannot per se be assumed to yield representative results for the general populations of countries based on a probabilistic sampling alone. Alternatives are discussed and-as for any survey-creative ideas on validating key results on indicators or hypotheses need to be developed and used. This will inevitably lead away from omnibus surveys to more focused studies requiring more complex methodological tools. While there may not be obvious solutions for every problem related to alcohol use prevention and policy or treatment use disorders, and it may take years to find solutions for some of the issues, continued use of the methodology of the status quo will surely fail to answer the questions posed by modern societies concerning these issues.
Congenital structural brain malformations have been described in patients with pathogenic phosphatase and tensin homologue (PTEN) variants, but the frequency of cortical malformations in patients with PTEN variants and their impact on clinical phenotype are not well understood. Our goal was to systematically characterize brain malformations in patients with PTEN variants and assess the relevance of their brain malformations to clinical presentation.
We systematically searched a local radiology database for patients with PTEN variants who had available brain magnetic resonance imaging (MRI). The MRI scans were reviewed systematically for cortical abnormalities. We reviewed electroencephalogram (EEG) data and evaluated the electronic medical record for evidence of epilepsy and developmental delay.
In total, we identified 22 patients with PTEN pathogenic variants for which brain MRIs were available (age range 0.4-17 years). Twelve among these 22 patients (54%) had polymicrogyria (PMG). Variants associated NEUROL 2020;881153-1164.

Sonographic measures of PFMC, i.e. bladder neck (BN) cranioventral shift and levator hiatal anteroposterior (AP) diameter reduction, were measured offline using ultrasound volumes obtained at rest and on maximal pelvic floor contraction.
