Introduction To address the question as to which infection surveillance measures are used during the ongoing COVID-19 pandemic in Germany and how they differ from pre-existing approaches. Methods In accordance with the systematic approach of a scoping review, a literature search was conducted in national and international medical literature databases using a search string. The search in the databases was limited to the period from 01.01.2000 to 15.11.2020 and has been subsequently completed by hand search until 08.03.2021. A hand search, even beyond 15.11.2020, seemed necessary and reasonable, since due to the dynamics of the ongoing COVID-19 pandemic, a large number of articles and regulations are being published very quickly at short notice. Results The literature search resulted in the following number of hits in the databases listed below PubMed 165 articlesCochrane 1 review and 35 studiesWeb of Science 217 articlesRobert Koch Institute 49 articles Thus, a total of 467 hits were identified, with a total of 124 hits being duplicates. From these, 138 articles were considered relevant to the COVID-19 infection surveillance situation in Germany based on established criteria. After reading the full texts, 92 articles and websites were ultimately included in the scoping review. Discussion Many of the lessons learned from previous outbreaks seem to have been implemented in the infection surveillance measures during the ongoing COVID-19 pandemic in Germany. Most of the changes compared with previous measures were based on technological streamlining of existing procedures and changes and more inclusion of the population in different infection surveillance measures.Breast carcinoma metastasis can involve any ocular structures, but involvement of the optic nerve is extremely rare. Choroidal metastasis is usually multifocal as well as bilateral and occurs late. We report an unusual initial presentation of metastasis from breast cancer; unilateral infiltrative optic neuropathy with concurrent choroid metastatic deposits in an adequately treated middle-aged female. Our present case, wherein for the first time in the literature, we illustrated unilateral infiltrative optic neuropathy and choroidal metastatic deposits secondary to breast carcinoma, will increase our knowledge about the various potential ocular presentations of this relatively common malignant disease.Most of the technically important properties of nanomaterials, such as superparamagnetism or luminescence, depend on the particle size. Previous studies on this topic are restricted by limited coverage of local time, latitude, and season, leaving a significant knowledge gap in the distribution of the topside ionospheric composition. In this study, we statistically investigate the O+ fraction measured by ICON/IVM over all the local time sectors and seasons at low/midlatitudes. For the first time, we have found that an isolated peak in the O+ fraction emerges in the post-noon equatorial region. The peak is most prominent during equinoxes, while during solstices it is connected to the O+ fraction bulges in the local summer midlatitudes. Simulations with SAMI3 coupled with thermospheric parameters from WACCM-X reproduce the peak of the O+ fraction. The post-noon equatorial peak can be explained by the net vertical motion of plasma consisting of transports either parallel or perpendicular to geomagnetic field lines.In near-Earth space, variations in thermospheric composition have important implications for thermosphere-ionosphere coupling. The ratio of O to N2 is often measured using far-UV airglow observations. Taking such airglow observations from space, looking below the Earth's limb allows for the total column of O and N2 in the ionosphere to be determined. While these observations have enabled many previous studies, determining the impact of non-migrating tides on thermospheric composition has proved difficult, owing to a small contamination of the signal by recombination of ionospheric O+. New ICON observations of far UV are presented here, and their general characteristics are shown. Using these, along with other observations and a global circulation model we show that during the morning hours and at latitudes away from the peak of the equatorial ionospheric anomaly, the impact of non-migrating tides on thermospheric composition can be observed. During March - April 2020, the column O/N2 ratio was seen to vary by 3 - 4 % of the zonal mean. By comparing the amplitude of the variation observed with that in the model, both the utility of these observations and a pathway to enable future studies is shown.Coincident Ionospheric Connections Explorer (ICON) measurements of neutral winds, plasma drifts and total ion densities (=Ne, electron density) are analyzed during January 1-21, 2020 to reveal the relationship between neutral winds and ionospheric variability on a day-to-day basis. Atmosphere-ionosphere (A-I) connectivity inevitably involves a spectrum of planetary waves (PWs), tides and secondary waves due to wave-wave nonlinear interactions. To provide a definitive attribution of dynamical origins, the current study focuses on a time interval when the longitudinal wave-4 component of the E-region winds is dominated by the eastward-propagating diurnal tide with zonal wavenumber s = -3 (DE3). DE3 is identified in winds and ionospheric parameters through its characteristic dependence on local solar time and longitude as ICON's orbit precesses. Superimposed on this trend are large variations in low-latitude DE3 wave-4 zonal winds (±40 ms-1) and topside F-region equatorial vertical drifts at periods consistent with 2-days and 6-days PWs, and a ~3-day ultra-fast Kelvin wave (UFKW), coexisting during this time interval; the DE3 winds, dynamo electric fields, and drifts are modulated by these waves. Wave-4 variability in Ne is of order 25%-35%, but the origins are more complex, likely additionally reflecting transport by ~20-25 ms-1 wave-4 in-situ winds containing strong signatures of DE3 interactions with ambient diurnal Sun-synchronous winds and ion drag. These results are the first to show a direct link between day-to-day wave-4 variability in contemporaneously measured E-region neutral winds and F-region ionospheric drifts and electron densities.Vaccine-induced immune thrombotic thrombocytopenia (VITT) is a rare complication after vaccination of Oxford-AstraZeneca coronavirus disease 2019 (COVID-19) vaccine (AZD1222) or Janssen COVID-19 vaccine. It makes a rare complication of thrombosis at common and/or uncommon organs with thrombocytopenia after COVID-19 vaccination four to 28 days later and most patients were younger than 60 years of age. We reported the case of a 75-year-old female with end-stage renal disease who received regular hemodialysis. She received Oxford-AstraZeneca COVID-19 vaccination eight days ago and then she suffered from intermittent chest tightness and epigastric pain with tarry stool passage for two days. Severe thrombocytopenia with elevated D-dimer value was noted and computed tomography of the chest showed azygos vein thrombosis. Elevated cardiac enzyme with ST-T change in 12-lead electrocardiogram was also noted. For positive anti-platelet factor 4 antibodies, VITT with myocardial infarction and azygos vein thrombosis was diagnosed.Primary cardiac tumors (PCTs) are rare and represent a heterogeneous group of tumors, potentially arising from various parts of the heart. The majority of these tumors are benign (90%), with myxoma being the most common subtype. Cardiac hemangiomas are rare vascular tumors that constitute 1-2% of all benign heart neoplasms. We present a rare case of a 79-year-old woman presented with multifocal embolic brain infarcts secondary to cavernous hemangioma of the mitral valve (MV). Cavernous hemangioma was successfully resected with follow-up imaging at six months, demonstrating no regrowth. There are no pathognomonic signs or findings to suggest cavernous hemangioma of the MV on clinical examination or imaging studies. Surgical resection and histopathologic analysis remain the gold standard for diagnosis and treatment, respectively. Following complete resection, the prognosis is generally favorable with a low recurrence rate, but periodic echocardiography is recommended to detect any potential recurrence.Background Negative pressure wound therapy (NPWT) has been shown to promote the healing of acute and chronic wounds. In our previous study, we demonstrated that a near-circumferential NPWT dressing provided "lift-off" on an in-vitro extremity model resulting in decreased pressure. We hypothesized that this decrease in pressure may increase perfusion distal to the NPWT dressing by increasing lymphatic drainage and venous flow. Methods In this study, we tested if a near-circumferential NPWT dressing caused any appreciable skin movement around the dressing. We then used a thermal imaging camera to test if there was an increase in perfusion to the foot when a near-circumferential NPWT dressing was placed around the lower leg and tested at various negative pressures. Finally, we wanted to see if an artificial "lift-off" mechanism would lead to an increase in perfusion. Results The skin was noted to stretch between the short ends of the NPWT dressing, consistent with our previously described "lift-off" mechanism. However, there was no correlation between negative pressure and perfusion to the foot in the other experiments. Conclusion This study demonstrated that a near-circumferential NPWT dressing may not have any appreciable effects on perfusion when applied on a healthy patient, however, future studies are needed to determine if similar results would be seen on a traumatized or otherwise compromised extremity.Corticosteroids have several widely documented adverse effects. However, there is no systematic study evaluating the frequency and associations of corticosteroid-induced mania. We report a case of corticosteroid-induced mania in a patient that previously tolerated higher doses of steroid therapy without neuropsychiatric symptoms. Although there is evidence suggesting a dose-dependent relationship, previous tolerance has not been proven to correlate with reduced frequency of developing mania or psychosis on the reintroduction of the medication. Therefore, patients restarting steroids should proceed carefully, even when receiving low doses.
