Prediction effects of these amino acid substitutions on the structure dynamics of the spike protein indicated a positive ΔΔG values and negative ΔΔSVib in most cases which is associated to structural stabilization and flexibility reduction of the S protein. Mutations E484K, N501Y and K417N belong to several SARS-CoV-2 variants of concern such as Alpha, Beta, Gamma and Delta, and showed high incidence among Brazilian isolates. These mutations have been described to increase RBD affinity to ACE-2 host and abolishment of RBD affinity to potent neutralizing ant-RBD. The increase in rates of infection and reinfection requires continuous genomic surveillance studies in order to characterize emerging mutations and monitor vaccine efficacy, and thus consideration structural data and dynamics in the observed phenotypes.Communicated by Ramaswamy H. Frequent Attenders with Medically Unexplained Symptoms (FA/MUS) are common in primary care, though challenging to identify and treat.
This study sought to compare FA/MUS to FA with organic illnesses (FA/OI) and the general clinic population (Non-FA) to understand their demographic characteristics and healthcare utilisation patterns.
For this retrospective, observational study, Electronic Medical Records (EMR) were obtained from Clalit Health Services, regarding the population of a sizeable primary care clinic in Be'er-Sheva, Israel. Electronic medical records were screened to identify the top 5% of FA. FA were stratified based on whether they had OI. FA without OI were then corroborated as having MUS by their physicians. Demographics, healthcare utilisation and costs were analysed for FA/OI, FA/MUS and Non-FA.
Out of 594 FA, 305 (53.6%) were FA/OI and 264 (46.4%) were FA/MUS. FA/OI were older (69.1 vs. 56.4 years,
<.001) and costlier (ILS27693 vs. ILS9075,
<.001) than FA/MUS. Average costs for FA/MUS were over four times higher than Non-FA (ILS9075 vs. ILS2035,
<.001). The largest disparities between FA/OI and FA/MUS were in hospitalisations (ILS6998 vs. link2 ILS2033) and surgical procedures (ILS8143 vs. ILS3175). Regarding laboratory tests, differences were smaller between groups of FA but significantly different between FA and Non-FA.

The goal of the current research is to subject the MRI to rigorous psychometric analysis using item response theory to answer three broad questions. Do the individual items of the MRI fit the scale well? Does the overall function of the MRI match what is predicted? Finally, do men and women respond similarly to the MRI? Using a graded response model, it was found that all but two of the items fit acceptable model patterns. Test information function analysis found that the scale acceptably captures individual differences for participants with a high degree of mate retention but the scale is lacking in capturing information from participants with a low degree of mate retention. Finally, discriminate item function analysis reveals that the MRI is better at assessing male than female participants, indicating that the scale may not be the best indicator of female behavior in a relationship. Overall, we conclude that the MRI is a good scale, especially for assessing male behavior, but it could be improved for assessing female behavior and individuals lower on overall mate retention behavior. It is suggested that this paper be used as a framework for how the newest psychometrics techniques can be applied in order to create more robust and valid measures in the field of evolutionary psychology.
Lack of physical activity represents a risk factor for both cardiovascular and chronic venous diseases (CVD), nevertheless a specific exercise protocol for CVD patient is still missing. This investigation was aimed to assess the impact of a standardized exercise protocol in a thermal water environment on physical fitness and quality-of-life (QoL) in CVD patients.
Sixteen (16) CVD patients performed 5 standardized exercise sessions in a thermal water pool. Before starting the exercise protocol, the cohort filled International Physical Activity Questionnaire (IPAQ) to determine their physical activity level. At baseline and at the end of the exercise program, leg volume, QoL, musculoskeletal and cardiovascular physical fitness were assessed by means of water plethysmography, validated questionnaire and functional test, blood pressure and heart rate at rest were also reported.

Diffuse pagetoid SCCIS of the esophagus is extremely rare with only two cases published in the English literature. In this article, we report a rare case of diffuse pagetoid SCCIS of the esophagus in an 89-year-old female with no significant past medical history who presented with dysphagia. Endoscopic examination of the esophagus was remarkable for multiple clean base ulcers spanning 4 cm in the proximal esophagus. Biopsy showed enlarged and hyperchromatic dysplastic cells in the basal half of the epithelium with scattered large individual pagetoid cells as well as several apoptotic dyskeratotic cells in the superficial half of the epithelium. Immunohistochemically, the dysplastic cells were positive for CK7 and p40 with overexpression of p53, and were negative for cytokeratin 20, SOX10, GATA3, CDX2, TTF1. Kreyberg stain was negative for mucin. The histologic features and immunohistochemical profile supported the diagnosis of esophageal diffuse pagetoid SCCIS.
To describe a new technique for sutureless and glue-free amniotic membrane transplantation (AMT) and to investigate its effectiveness to treat corneal persistent epithelial defects (PEDs), compared to bandage contact lens (BCL) application alone.
We performed AMT with "contact lens sandwich technique" (CLS-AMT) in 8 consecutive patients with central/para-central (up to 4.00 mm from the geometrical centre) PED/ulceration and we retrospectively compared the results with 11 BCL procedures.
The procedures were performed successfully with no complications.CLS-AMT showed significantly shorter healing time than BCL (24.0  ±  19.1 vs 42.9  ±  14.6 days; P < 0.05, Mann-Whitney test). Recurrence rates were 12% and 27% for CLS-AMT and BCL, respectively.
CLS-AMT technique, based on the suction effect due to the superposition of a bandage contact lens on the AM-ring complex, represents a quick, low cost, easy to perform and nearly non-invasive AMT technique. This approach is able to provide adequate fixation of AM, and it seems to be a safe and effective treatment for patients with PEDs.

To provide increased stability and better kinematics, more resembling a natural knee, a medial congruent (MC) tibial insert has been introduced. To investigate potential impact on implant migration, we evaluated the cemented Persona TKA, randomizing between MC and cruciate-retaining (CR) tibial designs.Patients and methods - 60 patients with primary osteoarthritis were randomized to either the CR or MC tibial component. Radiostereometric analysis (RSA) examinations, evaluating translational and rotational migration as well as maximal total point motion (MTPM), were performed on the first postoperative day, at 3 months, and 1 and 2 years after surgery. Preoperative, and 1- and 2-year patient-reported outcome measures (PROMs) data (KOOS and FJS) were collected.Results - The mean tibial MTPM at 3 months were 0.48 mm (95% CI 0.37-0.58) and 0.56 mm (0.45-0.67) for the CR and MC respectively. 2 years postoperatively the respective values were 0.62 mm (0.50-0.73) and 0.73 mm (0.49-0.96). There was no statistically significant difference in migration between groups, for either the femoral or the tibial component, regarding x-, y-, and z-translations or rotations.Both CR and MC groups improved as expected in PROM scores pre- to postoperatively, but without a statistically significant difference between groups. There were no revisions or other serious adverse events related to surgery.Interpretation - The results are promising, indicating good fixation for both designs, and this is in line with other well-performing TKAs on the market. The increased medial congruity of the MC inlay does not seem to affect the migration or the PROMs up to 2 years.
