It encompasses the assessment and management of symptoms and changes in functional status, the provision of advance care planning and goals of care discussions, prognostication and caregiver support. PC is applicable across the spectrum of cirrhosis regardless of transplant eligibility. Although a common misconception, PC is not synonymous with hospice care. Unfortunately, despite a high symptom burden and challenges with predicting disease course and mounting evidence to support the benefits of PC in patients with cirrhosis, comprehensive PC and referral to hospice are carried out infrequently and very late in the course of disease. In order to meet the needs of our increasingly prevalent cirrhosis population, it is important that all clinicians who care for these patients are able to work together to deliver PC as a standard of care. To date there are limited guidelines/guidance statements to direct clinicians in the area of PC and cirrhosis. Herein we present an evidence-based review of ten Best Practice Advice statements that address key issues pertaining to PC in patients with cirrhosis.

) on occurrence of acute gout flares in the general population and identify susceptible groups accordingly.
The data of emergency department (ED) cases with acute gout flare in Incheon city, Korea between January 1st 2008 and December 31st 2015 were collected from the National Health Insurance Service claims data. The levels of PM
and meterological measurements were provided by the Ministry of Environment and the National Meterological Office, respectively. To estimate the risk of daily ED visits due to acute gout flare, these time-series data set were analyzed using generalized additive models with Poisson distribution, including daily average PM
level, temperature, relative humidity, day of the week, national holiday, season, and date.
The risk of daily ED visits for acute gout flares per interquartile range increment of the average daily PM
levels significantly increased in the cumulative lag 0-7 model (relative risk, 1.018; 95% confidence interval, 1.008-1.027, P<0.001). In particular, men aged≥40 years and those with a history of diabetes mellitus or gout were significantly at a high risk of acute gout flares by subgroup analysis.

In radiation therapy (RT), due to their fast attenuation, kV x-rays are typically only used for superficial irradiation of skin cancer and for intra-operative RT (IORT). Recently, however, a number of kV RT techniques have emerged. In this review article, we provide a brief overview of the use of kV x-rays for RT. Various kV x-ray source technologies suitable for RT, such as conventional x-ray tubes as well as novel x-ray sources, are first described. This x-ray source section is then followed by a section on their implementation in terms of clinical, veterinary and preclinical applications. Specifically, IORT, superficial RT and dose enhancement with iodine and gold nanoparticles, as well as microbeam RT and FLASH RT are discussed in this context. Then, a number of kV x-ray RT applications in modeling and proof-of-principle stages, such as breast external beam RT with rotational sources, kilovoltage arc therapy and the BriXS Compton pulsed x-ray sources, are reviewed. Finally, some clinical and economic considerations for the development of kV RT techniques are discussed.Bedřich Roth was a Czech neurologist who dedicated his professional life to the study of sleep medicine. Shortly after WW2 he initiated a research program with an exclusive focus on narcolepsy and the hypersomnias. At the young age of 38 years, he published his first book. It was in Czech but had extensive English and Russian summaries. In 1967 he spent a study period in Marseille with Professor Henri Gastaut, who was very impressed by this bright and hardworking young neurologist. Dr. Roth, with his colleague Sona Nevšímalová, was the first to publish on the high frequency of depression in patients with narcolepsy and hypersomnia. He was the first person to define Idiopathic Hypersomnia and, along with Allan Rechtschaffen, to document the high frequency of sleep drunkenness in such patients. His many contributions, and his 65th birthday, were celebrated in 1984. Dr. Roth developed Hodgkin's lymphoma and passed away in November 1989. A conference celebrating his life and career took place in Prague in 2009. In 2015 the annual worldwide Idiopathic Hypersomnia Awareness Week organized by Michelle Chadwick celebrated the 35th anniversary of his 1980 book "Narcolepsy and Hypersomnia". Bedřich Roth should therefore be considered to be the true father of sleep medicine. A 4-year-old male dog, Blue Heeler, owas examined because of a reddish nodular mass in the right eye. A complete ophthalmic exam was conducted, including slit-lamp biomicroscopy, applanation tonometry, fundoscopy, ocular and abdominal ultrasound, hemogram, serum biochemistry profile, and a thoracic X-ray. #link# After the ophthalmic evaluation of the right eye, conjunctival hyperemia and the presence of a .5 cm diameter mass was found in the cornea. The diagnosis of fibrosarcoma was confirmed by histopathological analysis. The keratectomy showed satisfactory results in the removal of the corneal fibrosarcoma. The patient was followed up on for 18 months after the surgical procedure, and there were not any signs of local recurrence or metastasis.Recent studies have reported that vitamin D3 (VD) regulates ovarian function under physiological and pathological conditions. link2 Due to a lack of information concerning the expression of VD-related molecules (receptors VDR, PDIA3, and metabolic enzymes CYP27B1, CYP24A1) in the porcine ovary, this research aimed to determine the mRNA expression, protein abundance and localization of VDR, PDIA3, CYP27B1 and CYP24A1 in small (SFs), medium (MFs) and large (LFs) antral follicles of sexually mature gilts. We also examined the concentration of active VD in the follicular fluid of SFs, MFs and LFs, and the effect of 1,25(OH)2D3 on their steroidogenic activity in vitro. In the present study, we have demonstrated for the first time the patterns of VDR, PDIA3, CYP27B1 and CYP24A1 immunolocalization in porcine antral follicles of different sizes. Furthermore, the expression of VD-related molecules was influenced by follicle developmental stage. VDR and PDIA3 mRNA expression and protein abundance decreased with the follicle size they were the greatest in SFs, and the lowest in LFs. CYP27B1 mRNA expression was the highest in MFs and differed from that in SFs, whereas protein abundance was greater in MFs and SFs than in LFs. The expression of mRNA for CYP24A1 was higher in MFs than in SFs and LFs, while protein abundance did not differ between follicle classes. We have also described changes in the concentration of 1,25(OH)2D3 in the follicular fluid of antral follicles with its highest level in MFs. These findings show that the porcine antral follicle is a target tissue for direct VD action and is a local site of VD metabolism. Furthermore, we found that 1,25(OH)2D3 increased the secretion of progesterone and estradiol-17β by SFs and MFs in vitro, implying a crucial role of VD in the regulation of ovarian steroidogenesis in mature gilts. Therefore, VD appears to be an important intraovarian factor that could regulate follicular development and function in pigs.
Survivin is an inhibitor of apoptosis that is proposed as a target for anti-cancer therapy because of its high expression in cancer cells. It has potential as a prognostic and predictive biomarker of response to radiation and systemic therapies. We report its expression in head and neck squamous cell carcinoma (HNSCC) and its correlation with treatment response and survival.
We measured survivin protein expression in tumor specimens from 96 patients with HNSCC treated at Fox Chase Cancer Center, of whom 21 were p16+. Quantitative automated immunofluorescence was employed to score nuclear and cytoplasmic survivin in 5 tissue microarrays (TMAs) consisting of 316 H&N tumor cores and 107 control tissue cores. Survivin levels were then correlated to therapy response and survival outcomes.
Using the median score as the cutoff, overall survival (OS) was significantly shorter for the group expressing higher survivin in nuclear (p=0.013), cytoplasmic (p=0.018) and total compartments (p=0.006). No correlation was seen between survivin expression and patient sex or grade of tumor, T or N stage, or p16 status. Survivin expression in metastases did not significantly differ from that in primary tumors. Levels of p53 expression showed a significant positive correlation with higher survivin expression in the cytoplasm (p=0.0264) and total compartments (p=0.0264), but not in the nucleus (p=0.0729).

An increase in TBX5 dosage also leads to HOS, suggesting that TBX5 is a dose-sensitive transcription factor that needs to be tightly regulated but the molecular mechanisms involved remain unclear. In this work we report the cloning and functional analysis of human TBX5 promoter region 1 (upstream of exon 1) and promoter region 2 (upstream of exon 2), that probably regulate the transcription of the different transcript variants. In silico analysis showed several binding sites for cardiac and skeletal related transcription factors (TFs) and their functionality was assessed using promoter-luciferase constructions and TF-expressing vectors. MEF2A (Myocyte enhancer factor 2 A) was shown to positively regulate both TBX5 promoters, while EGR1 (early growth response 1) repressed both promoters.
