Recently, there has been an increase in robotic-assisted spine fusion for degenerative spondylosis of the lumbar spine. We present the case of a 60-year-old female with grade 1 spondylolisthesis at L4/5 and L5/S1 who underwent L4-S1 anterior lumbar interbody fusion (ALIF) with percutaneous robotic-assisted pedicle screw fixation. We provide a detailed analysis of the procedure including the speed of robotic screw placement and pitfalls of this surgical approach.Gastrointestinal stromal tumors (GIST) can occur anywhere in the gastrointestinal tract, with the primary site being the stomach. GISTs are often discovered incidentally on imaging due to the non-specific symptoms they present at the presentation. They can be distinguished from other mesenchymal tumors by immunohistochemistry staining. These tumors can range from benign to highly malignant, with surgical resection as the main treatment modality. Here, we present a case of a large GIST found not in the stomach, but in the mediastinum, incidentally on imaging, in a middle-aged male.Background Glomerulonephritis is among the most common and serious non-communicable diseases in the world, and some of them are major causes of chronic kidney disease, which eventually leads to kidney failure. In developing countries, it is the most common cause of end-stage renal disease (ESRD). Chronic kidney disease affects 10-16% of the adult population in Asia, including 21.2% in Pakistan. This study aims to determine the frequency and spectrum of glomerulonephritis at our center. Methodology This is a cross-sectional analysis of 81 renal core biopsies obtained between August 2020 and August 2021. The histopathological reports, demographic data, and relevant laboratory investigations, such as blood urea and creatinine levels, were collected. All cases were blindly reviewed by two pathologists with a special interest in medical renal pathology. Data were analyzed using SPSS version 22 (IBM Corp., Armonk, NY, USA). Results The majority of the patients (46.9%) were between the ages of 21 and 40 years. There was a slight male predominance, with 44 (54.3%) of the 81 participants being male. Hematuria was reported in 20 (24.7%) patients, with mean serum urea and creatinine levels of 75 mg/dL and 2.6 mg/dL, respectively. Nephrotic syndrome was the most common indication for a renal biopsy, accounting for 54 (67.7%) of the 81 cases. Chronic glomerulonephritis is classified into two categories, namely, primary and secondary glomerulonephritis. Focal segmental glomerulosclerosis (FSGS), the leading cause of primary glomerulonephritis, was found in 25 (30.9%) 81 patients. Furthermore, lupus nephritis (9.9%) and advanced glomerulonephritis (9.9%) were found in equal proportions among secondary glomerulonephritis. Conclusions According to our study, nephrotic syndrome was the most common indication for medical renal biopsy, with FSGS being the most common primary glomerulonephritis. Lupus nephritis and advanced glomerulonephritis were the most common secondary glomerulonephritis diagnoses.Fibrous uterine polyps are very common in women during or after menopause, and less often seen in women of child-bearing age, with a maximum of frequency between 40, and 49 years. We report a rare case of cervical localization of a pedicled fibrous polyp issuing from the cervix, in a 44-year-old female patient with 3 living children and a history of miscarriage, who had been presenting breakthrough bleeding, and pelvic pain for 3 months. An accurate differential diagnosis has to be done according to the patient's age and gender. Spontaneous round ligament hematoma is an uncommon cause of acute pelvic pain in adult female patients. To the best of our knowledge, it has never been reported in the literature in the paediatric population. Ultrasound examination is the first line imaging modality for pelvic pain evaluation in young women but it might result inconclusive. Thanks to its panoramic view and multiparametric approach, the MRI can play a pivotal role in the diagnosis of spontaneous round ligament hematoma in paediatric female patients, resulting in a more effective patient's therapeutic management.Gallbladder volvulus is a rare entity. It has been attributed to an elongated gallbladder mesentery, predisposing the gallbladder to twisting, obstructing the cystic duct and vessels, thus leading to ischemia and gangrene. Preoperative diagnosis can be elusive, but radiological features across multiple modalities have been described in the literature. We report a case of gallbladder volvulus in which the gallbladder appeared to be left-sided based on imaging, and present the radiological findings in keeping with a volvulus. Unlike cholecystitis, the treatment for volvulus is prompt detorsion and cholecystectomy; thus, accurate and timely diagnosis is paramount.The current imaging gold standard for detecting paradoxical diaphragm motion and diagnosing hemidiaphragm paralysis is to perform the fluoroscopic sniff test. The images are visually examined by an experienced radiologist, and if one hemidiaphragm ascends while the other descends, then it is described as paradoxical motion, which is highly suggestive of hemidiaphragm paralysis. However, diagnosis can be challenging because diaphragm motion during sniffing is fast, paradoxical motion can be subtle, and the analysis is based on a 2-dimensional projection of a 3-dimensional surface. This paper presents a case of chronic left hemidiaphragm elevation that was initially reported as mild paradoxical motion on fluoroscopy. After measuring the elevations of the diaphragms and modeling their temporal correlation using Gaussian process regression, the systematic trend of the hemidiaphragmatic motion along with its stochastic properties was determined. When analyzing the trajectories of the hemidiaphragms, no statistically significant paradoxical motion was detected. This could potentially change the prognosis if the patient was to consider diaphragm plication as treatment. The presented method provides a more objective analysis of hemidiaphragm motions and can potentially improve diagnostic accuracy.Clear cell meningiomas (CCM) are a very rare histologic subtype of meningioma usually affecting younger patients. The reported data on spinal CCM are extremely rare. Until today, only 89 cases have been reported. Furthermore, CCM without dural attachment is even rarer since only 19 cases have been reported in English literature. In this article, we present the twentieth case of a spinal CCM without dural attachment. Our patient was a 58-year-old female who was presented with pain in her lower back and bilateral sciatica for 6 months. Magnetic resonance imaging showed an intra-dural well-demarcated lesion at L3. Via a posterior approach, total resection was possible due to the lack of dural adhesion of the tumor. ADEM can develop following infection or vaccination. Here, we present a 37 y/o male patient with progressive muscle weakness in all limbs along with dysphagia following COVID-19 vaccination. Brain magnetic resonance imaging (MRI) revealed typical imaging findings which presented as multifocal T2-FLAIR signal changes in the corticospinal tract, pons, and temporal lobe with diffusion restriction. This anomaly is typically asymptomatic unless associated with other conditions. We report a case of separate origins of the left internal and external carotid arteries from the aorta in a patient with intracerebral hemorrhage. A 42-year-old man was transferred to our hospital by ambulance because of left hemiparesis. Computed tomography scan revealed right putaminal hemorrhage. Computed tomography angiography and digital subtraction angiography demonstrated independent origins of the left internal carotid artery and external carotid artery from the aortic arch. We hypothesized that people higher in agentic grandiose narcissism would help during the COVID-19 quarantine for egoistic reasons and that communal grandiose narcissists would help because they care for the people in the community. Because vulnerable narcissists tend to self-protect, we expected them to be less helpful overall and report helping to either make themselves feel better or avoid social disapproval. We found supporting evidence such that agentic grandiose narcissists reported helping in a greater number of ways because they were motivated by self-promotion while communal narcissists helped in more ways because they were motivated by concern for others. Vulnerable narcissists reported doing less to help others and a lack of concern for others. Further, we found that the extent to which grandiose narcissists communicated more frequently about their various helping efforts was associated with increased state grandiose narcissism. In this research, we present a new genome-guided transcriptome assembly algorithm, namely Tiglon, which integrates multiple alignments of different mapping tools and builds the labeled splice graphs, followed by a label-based dynamic path-searching strategy to reconstruct the transcripts. We evaluate Tiglon on a simulated dataset and 12 real datasets under the Hisat2 and Star mappings. The results indicate that the integrating techniques of Tiglon exhibit great superiority over the state-of-the-art assemblers, including StringTie2 and Scallop, depending on Hisat2 alignments, Star alignments, or the merged alignments of both. Especially, Tiglon is significantly powerful in recovering lowly expressed transcripts.
