BACKGROUND AND AIM Chronic inflammation has an important role in the development and progression of type 2 diabetes through immunologic inflammatory mechanisms. Simple new inexpensive inflammatory markers may contribute to the detection of microalbuminuria. Aim of our study is to evaluate the predictive value of neutrophil-to-lymphocyte ratio (NLR), mean platelet volume (MPV), and red blood cell distribution width (RDW) for microalbuminuria in type 2 diabetic patients for possible application as prognostic factors for the prediction of microalbuminuria and the progression of disease in patients with diabetes. METHODS A total of 168 patients with type 2 diabetes mellitus were classified into gender- and BMI-matched three groups according to hemoglobin A1c and microalbuminuria Group A 53 patients with controlled diabetes, Group B 57 patients with uncontrolled diabetes, both without microalbuminuria, and Group C 58 patients with uncontrolled diabetes with microalbuminuria. Levels of NLR, MPV, and RDW between the study groups were examined and compared. RESULTS A significant difference in NLR was found between Group C and groups A and B (P  less then  .001, P = .005, respectively). A statistically significant difference in RDW was found between groups B and C (P = .014). Receiver operating characteristic curve analysis of inflammatory markers and microalbuminuria prediction showed an area under curve (AUC) of 0.675 for NLR (CI 0.58-0.76, P  less then  .001) and 0.614 for RDW (CI 0.52-0.70, P = .013). NLR value of 2.54 has 39.7% sensitivity, 78.8% specificity, and 45% positive predictive value (PPV). RDW value of 14.44 has 37.9% sensitivity, 76% specificity, and 41.5% PPV. CONCLUSIONS Neutrophil-to-lymphocyte ratio and RDW have PPV for microalbuminuria in diabetic patients. © 2020 The Authors. Physical restoration of stream channels is widely used to offset the effects of urbanization on streams, with the goal of improving ecosystem structure and function. However, these efforts are rarely guided by strategic analysis of the factors that mediate the responsiveness of stream ecosystems to restoration. Given that ecological gradients from headwater streams to mainstem rivers are ubiquitous, we posited that location within a river network could mediate the benefits of channel restoration. We studied existing stream restorations in Milwaukee, Wisconsin, to determine (1) whether restorations improve ecosystem function (e.g., nutrient uptake, whole-stream metabolism) and (2) how ecosystem responses vary by position in the urban river network. All rights reserved.BACKGROUND Paediatric dentists in the United States may be at greater risk for occupational burnout and/or depression because of chronic stress associated with provision of paediatric dental care and increasing prevalence of females in the workforce. AIMS To determine the prevalence of occupational burnout and/or depression among US paediatric dentists. check details DESIGN A self-administered online anonymous survey was sent to members of the American Academy of Pediatric Dentistry (n = 4735). The questionnaire consisted of seven demographic items, 22 items of Maslach Burnout Inventory (Three subscales Emotional exhaustion, Depersonalization, and Personal accomplishment), and eight items of Patient Health Questionnaire-8. RESULTS The survey had a response rate of 11.4% (females = 53%). Twenty-three per cent of respondents had high emotional exhaustion while fewer respondents had high depersonalization (12%) or low personal accomplishment (10%). Nine per cent fulfilled the study's definition of occupational burnout (high emotional exhaustion + high depersonalization). Seven per cent of respondents had moderate-to-severe depression and showed significant correlations (P  less then  .05) with high emotional exhaustion, high depersonalization and low personal accomplishment. Two out of five respondents with occupational burnout also had moderate-to-severe depression. There were no gender differences in prevalence of burnout or depression. KGaA, Weinheim.Idiopathic aplastic pancytopenia is an uncommon disease in dogs which results in pancytopenia and for which an immune-mediated etiology is suspected. A small number of affected dogs reported in the veterinary literature have responded to immunosuppressive medication but the prognosis generally is considered poor with a reported mortality rate of 80%. Reported response rates to immunosuppression alone in affected people are low with overall and complete responses of 65 and 10%, respectively. With the addition of eltrombopag, an orally available thrombopoietin receptor agonist, reported overall and complete response rates in people increase to 94 and 58%, respectively. Herein, we report the use of eltrombopag in a dog with idiopathic aplastic pancytopenia. Eltrombopag was started after no response was seen to treatment with prednisolone and cyclosporine. Complete remission was achieved after the addition of eltrombopag and was sustained after stopping the medication. © 2020 The Authors.
